He/she will provide information and resources for clients to help them manage their basic daily needs, coordinate appointments between organizations. Provide education about community resources to clients and families. Duties and Responsibilities + Provide timely services as indicated in the program specifications and complete documentation requirements within the timeframes and content standards established by best practices + Provide education and support to clients so they can find solutions to their health and human service needs. This includes but is not limited to housing, employment, food assistance and medical needs + Work to develop and maintain a resource listing for the applicable service area + Develop case management objectives and a quarterly case plan collaboratively with the client and other resources if applicable. In addition, facilitate a quarterly Case Review Meeting, with all parties in attendance, to discuss the client’s case management objectives and plan for the next quarter + Assist in tracking information, beginning at intake and admission, through treatment and into aftercare, which becomes part of the statistical data bank of Samaritas. Track data as necessary for the program’s funding and PIHP or program requirements + Assure that all authorizations/reauthorizations or documentation/paperwork requirements are clinically appropriate and timely (according to contracts) for all county, State of Michigan, Court, and Managed Care Agencies + At client discharge, coordinate aftercare plans to ensure treatment continuity for the clients psychiatric, medical, and therapeutic care + Responsible for quality assurance compliance, including but not limited to participating in peer supervision, peer utilization reviews, compliance with service delivery criteria and reporting requirements for each individual served + Provide leadership and support for credentialing, paneling and contracting as applicable _________________________________________________________________________________ Job Qualifications Education, Training, and Licensure/Certification + Bachelor’s degree in Social Work, Psychology or another associated human service profession preferred + Minimum two years’ experience in the human service field preferred Knowledge Skills and Abilities + Ability to apply commonsense and understanding to carry out instructions furnished in written, oral or diagrammatic forms. + Ability to work constructively with supervisor and staff. + Familiarity with and prior experience with standard computer word process programs is required. + Ability to perform basic arithmetic operations, including addition, subtraction, multiplication and division. + Ability to maintain confidentiality with client, employee and organization information. + Ability and willingness to work as part of a team in achieving common goals. Ability to work constructively with staff and other stakeholders. + Ability to work effectively in a cross-cultural context. + Ability to speak, read and write fluent English. Strong oral and written communication skills with ability to prepare letters, summaries and reports using prescribed format and conforming to rules of punctuation, grammar, diction and style. + Ability to manage multiple functions of a complex organization.
