JOB DESCRIPTION:
SUMMARY OF POSITION
Responsible for assisting with the process surrounding placing a child in foster care. Provides for the safety, permanency and well-being of foster care clients, serving as the primary link between Centerstone, Department of Children/Families (DCS), foster families, biological families, schools, and services needed. Conducts in home visits and manages issues and concerns of foster children/families, provides linkage and referral to other services and supports to enhance family functioning. The position requires frequent driving to attend meetings, court, visit clients, families, etc. All mileage is reimbursed.
ESSENTIAL DUTIES & RESPONSIBILITIES
Learns and applies Centerstone and DCS Policies and Procedures. attends and participates in Case management/Foster care team meetings Links Clients families to community resources. Assis families with medication issues when necessary. Helps achieve all EPSDT and Dental requirements, including all necessary follow-up. Signs off on MAR in the foster home. Completes all TFACTS and Centerstone EMR requirements on time. Completes all required face to face visits per DCS policy. Completes all monthly summaries and enters into TFACTS (timely) Performs documentation within 48 hours of contact Develops plan of care within 30 days of admission and every 90 days; thereafter, complete with goals, objectives, interventions. Documents progress according to documentation expectations Uses behavior management and crisis management skills when needed Coordinates case management services with school personnelCoordinates, transports and supervises visitation as agreed upon in the CFTM. Assists foster parent/s with transportation when needed. Attends court as required. Coordinates community service projects when necessary. Completes required training and continuing education. Completes timesheets in a timely manner. Participates in all staff supervision activities. Maintains electronic record in Kaleaidacare Participates in 24/7 on-call coverageKNOWLEDGE, SKILLS & ABILITIES

Education Level
Bachelor’s degree required. Degree in Psychology, Social Work, Criminal Justice or other behavioral health care field preferred.

One to two years minimum experience with children, youth and families.
