Case Management Manager
**Work Shift:** Day Shift **Time Type:** Full time **Department:** CC017640 Case Management **Summary:** Case Management Under the direction of the Director Case Management, the Manager Case Management assists with planning, implementing and evaluating the strategic goals of the utilization management program. Oversees staff that process referrals, authorizations, billing, utilization review, and capitation for hospital services and assists in the daily operation of the discharge/utilization management department. Has some authority for personnel actions. Assist as a hospital liaison with community resources which are consulted for our patients to provide care after discharge. Coaches, mentors and supports staff in consistent delivery of quality patient and family-centered care. Promotes active involvement of staff in effective interdisciplinary communication, collaboration team rounds hand offs of team and clinical decision making. Guides team to achieve good care coordination with other disciplines and efficient delivery of care. Evaluates annually the discharge planning activities. Ensures appropriate follow up and reporting on occurrences and near misses. Assists the Director of the department with reports for UM committee. 2. Interviews and assists with hiring qualified staff. Evaluates issues contributing to staff turnover and negatively impacting retention. Provides oversight to successful completion of department orientation, onboarding, education, training and required annual competencies. Provides ongoing feedback to staff regarding their professional performance and completes annual performance reviews. Consoles, coaches, mentors and disciplines staff as appropriate for performance issues. Promotes positive work environment and fosters good teamwork. Covers for case managers when out on vacation, sick, FMLA as needed. 3. Maintains oversight of daily operations and staffing, ensuring appropriate service assignments for timely completion of care and discharge planning and collaborates to identify and address potential cost savings and improved efficiencies in work processes. 4. Fosters empowered decision-making, accountability, and autonomy for the professional nurses. Role models and promotes ongoing advancement of professional practice (e.g certification) and professional image of nursing of AC at work and in the community. Volunteers for activities which improve the overall health and wellbeing of children and families. 5. Performs other duties as assigned.
