Case Management Coordinator- Day Services Manager

PURPOSE:

The Case Management Coordinator is responsible for providing leadership, direction and training to the case management teams for community participants at all CDS sites including ensuring appropriate assignment of caseloads (e.g. matching staff strengths to participant needs) as well as overseeing the implementation strategy for person-centered planning. This role will directly supervise Case Managers at multiple locations.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

The list of essential duties and responsibilities, as outlined herein, is intended to be representative of the task to be performed. The omission of an essential function does not preclude management from assigning duties not listed herein is such duties are a logical assignment to the position.
1. Oversees the CDS case management teams in the organization of provision of services by coordinating the communication process with      the interdisciplinary team, the development of monthly review of goals and objectives, the selection of data collection, the proper use of          resource materials and equipment, and training of key individuals responsible for plan development and implementation.
2. Oversee core/therapeutic services that may include health and wellness, creativity, communication, quality relationships, and community         inclusion. Maintain an ongoing schedule of skills classes and activities for program outcomes.
3. Responsible for program’s adherence to all standards, regulations and guidelines mandated by regulations and funding bodies.
4. Effectively communicate all concerns and needs concerning the program to the direct supervisor, administrator and assigned staff.
5. Communicate regularly, contingent upon individuals’ consent, with family/guardian/designees to relate progress, achievements, concerns,       or unusual incidents.
6. Act as a liaison between the agency and family members, stakeholders, professional connections, and community resources.
7. Perform regular audit reviews for reporting compliance ensuring accurate documentation and filing of information in case records.
8. Serve as a resource to staff regarding processes related to the case management function (e.g. referral, assessment, progress review,           behavioral strategies).
9. Encourage professional growth of assigned staff through meetings, routing of materials, etc.
10. Responsible for the safety of individuals at all times, immediately reporting accidents and hazards for correction.
11. May assist individuals with personal care needs.
12. Advocate for individuals served and bring to light any barriers to independence.
13. Maintain confidentiality of all billing, program demographic and personal information per HIPAA and the Confidentiality Act.
14. Oversee the collection of accurate information which may include attendance, billing, payroll, program outcomes, and grant information          as requested.
15. Transport individuals to various program sites on an as needed basis.

17. Maintain reduced caseload as needed, serving as case manager for designated individuals and filling in for vacant case manager                  positions.
18. Hire, train, supervise and conduct performance reviews, corrective actions and separations for all case managers including job related          duties, best practices, policies and procedures.
19. Oversee the coordination of incident report documentation as it relates to policy, DHS regulations and CARF requirements including               review of reports, maintaining a database, and coordinating data collection for statistical outcomes.
20. Assist in the monitoring of recommended staff to client ratios including suggestions regarding additional support.
21. Establish and maintain relationships with funding and referral sources, and other providers in order to expand and enhance supports to          individuals served.
22. Maintain American Heart Association certification in First Aid and CPR.
23. Remain current on requirements for QIDP certification.
24. Successfully complete Little City required training, re-training, and any additional training as required by Little City policies and practices,       or as assigned by his/her immediate supervisor.
25. Other duties as assigned by supervisor.
