Summary: Supports critical and time sensitive functions of the Case Management team, including but not limited to managing post-acute care referrals, and communicating directly with patients/families, post-acute care agencies, and other key stakeholders. Proactively ensures patient is appropriately scheduled for clinical needs and connected to clinical, social, and community resources. Contacts payers and review agencies to initiate prior approval for treatment and supplies. Assists patients in determining coverage benefits with durable medical equipment and other supplies. The pay range for this position is $46,306 - $56,743 annually which includes a comprehensive benefits package.

Qualifications:Required Education and Experience
1. Associates degree in business or health-related field; or
2. High School diploma or High School Equivalency diploma and current New York State Certification as a Medical Assistant; or
3. High School diploma or High School Equivalency diploma and License to practice as a Licensed Practical Nurse in the State of New York; or
4. High School diploma or High School Equivalency diploma and the equivalent of two (2) years of full-time healthcare clerical or clinical experience.

NOTE:
Required degrees must have been granted by an accredited school, college or university or one recognized by Roswell Park Comprehensive Cancer Center as following acceptable educational practices.
