Position Summary:

Coordinates individual referrals from community partners and vendors, overseeing the intake process, including ASAM assessments, diagnostic determinations, and appropriate treatment placements. Provides ongoing support to participants and collaborates with referring community partners and vendors to ensure continuous client care and seamless service delivery.

Essential Job Functions:

Coordinates scheduling of Intake appointments with community partners/vendors  Work closely with community partners/vendors to ensure consistency in client care Ensure timely communication between individuals and healthcare providers Match participants with appropriate ASAM Counselors based on participants’ needs and needs of the referring community partner/vendor Maintain regular communication with community partner/vendor to address any concerns or issues Assist individuals with navigating the Placement process and accessing necessary resources Maintain accurate records of program placements, participant information, and community partner/vendor details Prepare reports and documentation as required or requested Develop and maintain relationships with community partners/vendors/organizations and placement providers
