The Nurse Case Manager (NCM) is responsible to manage a caseload of patients across the continuum of healthcare services and assure optimal quality outcomes, patient satisfaction, and appropriate use of resources. The NCM is responsible for directing an organized, patient-centered interdisciplinary team system that coordinates the delivery of healthcare services by continually monitoring, evaluating, and modifying the treatment plan to achieve optimal patient outcomes and efficacious use of resources. The NCM coordinates with physicians, Case Management Social Worker, and other health team members to expedite medically appropriate, cost-effective care and promote quality through evidence based medical practice throughout the health care system.

Required Qualifications:
Graduate of an accredited school of nursing Upon hire: Montana Registered Nurse License 3 years experience in Clinical Nursing 1 year Hospital case management experiencePreferred Qualifications:
