About Legacy Treatment Services: Legacy Treatment Services is a behavioral and mental health social services agency. Legacy providers services to over 20,000 adults, youth, and adolescents throughout 17 counties in New Jersey. Legacy’s mission is to change the behavioral health and social service outcomes for people of all ages from surviving to thriving. This is a 40 hour full-time benefitted position which includes some evenings required: 3.2 weeks of PTO in first year Medical, vision, dental & life insurance benefits 403(b) employee participation and employer match 9 Agency-paid holidays Salary - $40000 to $50000 depending on education and experience Provides care coordination in the Contingency Management and Other Strategies for the Treatment of Stimulant Use Disorders program. Maintains compliance with agency policies, applicable regulations, and relevant standards of care determined by accrediting/governing authorities. Plans, organizes, and implements comprehensive services to consumers. Demonstrates consistent and proficient knowledge, skills, and dispositions required to implement coordinated approach to the delivery of health, substance abuse, mental health, and social services, linking consumers with appropriate services to address specific needs and achieve stated goals: Implements care management services to consumers addressing and coordinating services identified. Demonstrates understanding of Contingency Management Model, theories of addiction, and related issues. Manages and distributes CMI to consumers who demonstrate positive behavior change consistent with meeting treatment goals. Completes timely DMHAS programmatic intake and follow-up surveys on each enrolled individual in the CM program. Submits timely monthly roster data and quarterly reports utilizing templates provided by DMHAS. Collect and report data timely of programmatic forms required at specific data collection points and ensure all data reported are accurate to meet SAMHSA obligations under GPRA. Recognizes the importance of family, social networks, community systems, & self-help in recovery Aware of insurance/ health maintenance options and assists consumers access those benefits. Renders care services through evidenced based strategies to engage and retain consumers in care; provides linkages/referrals; coordinates, monitors, follows up with care issues. Monitors consumer progress for changes in clinical symptoms and treatment side effects or complications. Accurately completes all required record keeping based on agency procedures. Maintains consumer confidentiality, adheres to related agency/program procedures. Must be able to assess and interpret data about the consumer’s status in order to identify each consumer’s needs and provide the appropriate care, including age-related care, needed by the consumer group. Demonstrates awareness, appreciation and respect of cultural differences. Adheres to established policies, ethical guidelines by relevant governing authorities and contracts. Participates in all designated meetings and supervision sessions as well as clinical practice activities. Demonstrates ability to meet the organization's expectations for productivity, quality, and goal accomplishment. Follows all PI, HR, budget, contract requirements pursuant to the daily operations related to program Education/Experience: Bachelor’s degree in health, psychology, counseling, social work, education, or other behavioral health profession with 2 years’ experience working in field of addictions. Must possess a valid driver’s license in good standing.
