Care Coordinator, Integrated Health Home

The Care Coordinator reports to the Integrated Health Home (IHH) Program Manager, and provides and arranges for the provision of services necessary to enable clients to achieve optimal whole health.  This position assists clients in identifying, coordinating, securing and sustaining resources necessary to live a healthy lifestyle and have positive whole health, including wellness, behavioral and physical health.

Generally Monday-Friday, 8am-4:30pm

Qualifications 

Bachelor's Degree in Human Services fieldExperience working on a multi-disciplinary teamAbility to effectively and professionally manage difficult client issuesStrong organizational, written and verbal communication skillsValid driver’s license, regular availability of personal car, and evidence of insurance coverageGood driving record as determined by BMC requirementsAbility to meet clients in their homes or in public places with ability to drive clients safely, and a willingness to transport clients in own vehicle as neededExperience working with Microsoft Office software
