PRIMARY FUNCTION
The Behavioral Health Care Coordinator (BHCC) is responsible for providing care coordination services to patients with mental illness and/or substance abuse who have high rates of behavioral health utilization and/or psychosocial vulnerability. The BHCC  focuses on connecting families with community partners and services, motivating families to complete assessments.

ESSENTIAL DUTIES AND RESPONSIBILITIES
This list may not include all the duties that may be assigned.
1)    Reviews clinical appropriateness and urgency of referrals.  Performs warm calls/wellness checks in between provider visits with patients to escalate any new clinical concerns to providers, monitor treatment compliance and patient status.
2)    Monitor care coordination and behavioral health consultation plan according to the patients’ needs.
3)    Is part of the team who assesses, plans, implements, monitors and evaluates community mental health services to meet the patient’s behavioral health needs.
4)    Assists patient by contacting external behavioral health/substance use community-based providers to assist with accessing quality and cost-effective care. Ensures we steer our patients to our in-network mental health specialty partners and aids in clinical assessment to match urgency and severity of the case.
5)    Collaborates with the patient and patient care teams, both internal and external, to ensure appropriate actions  (example: schedule appointments, follow up with family compliance) are taken to meet the patients’ needs. Any concerns are escalated to the internal team.  Maintains regular communication with external providers to ensure coordinated care.
6)    Addresses barriers to healthcare and or Social Determinants of Health including, but not limited to, homelessness, housing instability, financial resources, transportation, childcare, etc.  by using the social determinant of health screening tool . Coordinates with specialty community partner care manager to address barriers to care.  Use Social Determinats of Health vendor as needed and available.
7)    Serves as a source of behavioral health knowledge for internal staff.
8)    Demonstrates cultural sensitivity and respect for the patient.
9)    Maintain a caseload of a minimum of 100 patients.
10)    Perform other duties as assigned.

QUALIFICATIONS
EDUCATION: 
•    Minimum Bachelor’s degree in Psychology, Nursing or related field. 
EXPERIENCE: 
•    A minimum 2-3 year of healthcare experience required with experience in a behavioral health setting.

CERTIFICATION
•    Preferred emphasis in Psychology or behavioral sciences.

KNOWLEDGE, SKILLS AND ABILITIES
•    Strong telephonic skills and verbal articulation skills.
•    Ability to assess emergency situations and act accordingly. 
•    Possess a positive attitude and display exceptional customer service skills. 
•    Detail oriented and ability to multi-task.
•    Ability to build effective working relationships with patients, parents, guardians, fellow employees and the public.
•    Some clinical, interpersonal, organizational and communication skills.
•    English writing skills with spelling, grammar, and punctuation.
•    Basic Excel and knowledgeable in Microsoft Outlook, and Word.
•    Willing to learn the quality process as it relates to a medical practice. 
•    Computer skills and experience with electronic health records.  
•    Ability to work independently and as a team player.

TYPICAL WORKING CONDITIONS
•    Patient-facing (remotely or physical)
•    Must be U.S. based.
•    Full time remote requires quiet workspace without distractions or interruptions while interacting with patients and caregivers.
•    Considerable sitting on a daily basis
•    Able to work under emotionally stressed conditions.
•    Occasionally subject to challenging environment.

The expected base hourly range for this position located in Carmichael, CA starts at $31.14.  It is not typical for offers to be made at or near the top of the range. Pediatric Associates offers are based on several factors including but not limited to the candidate’s education, work experience, and certifications, all of which are dependent on the requirements of the specific role.  In addition to base salary, the Pediatrics Associates Family of Companies and it’s affiliates offers a comprehensive benefits package, potential incentive and recognition programs, and retirement planning (all benefits are subject to eligibility requirements based on position, and division).
