About the Role

As a Care Coordinator, you'll lead and inspire a dedicated care team, ensuring residents’ health needs are closely monitored and met with excellence. You'll take charge of documenting and addressing clinical challenges, collaborating with Anglicare and residents’ families to provide holistic support. This role is ideal for experienced leaders in nursing, care coordination, and similar fields, ready to make a meaningful impact.

Permanent Fulltime role

Involves shifts- Monday regular shift and Night shifts from Tue-Friday (classified as weekend shift) providing leadership to the care team.

Key Responsibilities:

Empathy & Respect: Treat residents with compassion, supporting their unique choices in daily life.

Effective Care Coordination: Ensure quality staff practices meet residents' clinical and personal needs.

Care Planning: Regularly review and update resident care plans.

Leadership: Lead with professionalism, aligning with the organisation’s Christian values.

Clinical Oversight: Ensure RNs, allied health staff, and doctors meet clinical standards.

Team Collaboration: Share clear information for effective multidisciplinary care.

Funding Accuracy: Maintain accurate documentation for funding compliance.

Team Support: Encourage teamwork, assist in reviews, and support RN selection.

Family Communication: Keep families informed and address concerns as needed.

Resident Admission: Oversee assessments to align admissions with care needs.

This role Involves night shifts- Monday is a regular shift and Night shifts from Tue-Friday (Fri is classified as weekend shift, which gives you additional allowances) and key responsibility is providing leadership to the care team.

About you:

Minimum 2 years’ experience as a Registered Nurse, AHPRA registered

Previous experience in Residential Care Services.

Strong communication skills (written and verbal) and ability to build rapport with clients and their families.

Ability to communicate effectively with all levels of staff and management.

Intermediate computer skills and experience working in an electronic environment.

Current NDIS Workers Screening Check
