Summary: Under the direction of the physician on the Cardiology service assists with and facilitates the care of patients on the cardiac service. Utilizes the nursing process to assess the clinical emotional. psychosocial and educational needs of the patient and family Supports the hospital nursing staff as an adjunct in the care of the patient by providing leadership and expertise m cardiac nursing.
Job Description:1. Generates list of patients at start of the day for each cardiology. Reviews any overnight issues; such as new admissions. planned discharges and any urgent issues with the Cardiovascular Medicine (CVM) physician at beginning of shift and throughout the day. Makes rounds with cardiology physicians to facilitates patient care.
2. Under the direction of the physician, triages patients by utilizing history and physical knowledge to formulate a plan of care and utilizes established standards of care. Provides for continuity of care throughout the patient s hospital stay. Communicates plan of care to the bedside nurse and the primary physician team. Writes plan on white board m patient's room. Coordinates the process of testing procedures and laboratory tests.
3. Assists in getting patient testing completed based on patient need. Assists in obtaining additional patient information as needed. Assists in obtaining consents for procedures.
4. Performs a risk stratification for Transfusion and Contrast Induced Nephropathy pr or to heart catheterization. Monitors results of tests and communicates abnormal results to appropriate physician
5. Facilitates teaching for cardiac testing such as stress testing and cardiac catechization. Will verbally review testing with patient and hand out education books in addition to assisting patient to watch educational video.
6. With the hospital staff. assists and reinforces the education of cardiac patients in cardiac disease. testing procedures, medications, discharge and the follow up process. Assesses the knowledge level through formal and informal methods, including use of the "Teach back" method to determine the patient's level of understanding Provides expanded resources as determined by the patients and family needs.
7. Makes follow-up appointments and provides prescriptions for the patient Cardiac rehabilitation information is provided as needed. Referral to home health care 1s done m collaboration with staff. May recommend community resources to meet patient and family needs Assures that an effective transition plan is in place prior to discharge.
8. Assists in the development of educational materials and protocols. specifically related to the care of the cardiac patient. Assists in research protocols and advises research staff of potential candidates Acts as a resource to the multi-disciplinary teams. May utilize workshops and/or seminars to increase professional knowledge.
9 Required to attend assigned committees or meetings 75% of time. Committee assignment will be reviewed by Nurse Manager annually.
10. Participates in initiatives such as discharge before 11 am, core
measures. reducing readmissions and Improving the patient experience
11 Work schedules must be flexible to address patient care needs
12. Checks work email on a regular basis with minimum of once weekly.
13 Promotes the standards of care and service excellence according to the philosophy and goals of Trinity Health.
14. Provides quality patient care by considering age, development and cultural needs through competent clinical practice.
15 Maintains good rapport and cooperative relationships Approaches conflict in a constructive manner. Helps to identify problems, offer solutions, and participate in their resolution.
16. Maintains the confidentiality of information acquired pertaining to patient. physicians, associates, and visitors to Trinity Health. Discusses patient and hospital information only among appropriate personnel in appropriately private places.
17. Behaves In accordance with the Mission, Vision and Values of Trinity Health.
18 Assumes responsibility for performance of job duties m the safest possible manner, to assure personal safety and that of coworkers. and to report all preventable hazards and unsafe practices immediately to management.
REQUIRED EDUCATION, EXPERIENCE AND CERTIFICATION/LICENSURE
Bachelor of Science in Nursing preferred; Associates degree in Nursing required. BSN degree preferred or must agree in writing to obtain a BSN within five years. Current RN licensure in the State of Michigan required. BLS required. Two years recent experience in direct patient care as an RN with demonstrated cardiac clinical expertise or equivalent experience.
