Summary
The Cancer Services Coordinator (CSC) supports and guides oncology patients through the cancer continuum. The CSC role is part of a multidisciplinary team approach at Goshen Center for Cancer and will serve as a liaison between the patient, community, and clinical care team. This role will facilitate the needed care at the appropriate time within the healthcare system, including coordinating the transition from inpatient to outpatient care along with identifying and addressing barriers of care along the way. The CSC will work collaboratively with the patient’s multidisciplinary care team to integrate clinical and non-clinical services and coordinate community resources and support. The CSC will navigate patients throughout their oncology journey from diagnosis, through treatment, and onto survivorship. A high degree of coordination is required and includes but is not limited to; appointment coordination, tracking completion of diagnostic testing, customizing services to meet the patient’s needs, and collaborating between the Center for Cancer Care and the community physicians referring to the Center for Cancer Care. This role will focus on expedited patient access to care by being the singular point of contact for patient’s clinical and non-clinical needs. This Colleague becomes the face of the Centers for Excellence model.
Position Qualifications
Minimum Education: Must be a High School graduate Preferred Education: Business or Junior College/secretarial/clerical training Minimum Experience: 3 years secretarial, receptionist or clerical experience. Computer experience in Word and Excel. Preferred Experience: Experience within an oncology program and familiarity with the process of patient flow from intake, diagnosis and treatment of cancer. Experience with improving patient satisfaction and service recovery. Management experience with oversight of peers and registered nurses. Formal professional mentoring experience. Knowledgeable in MS office applications and email tools.
