Inova Fairfax Hospital is seeking a dedicated Bone Marrow Transplant Clinical Pharmacy Specialist to join the team. This role will be full-time day shift from Monday – Friday, various shift| Sign-on Bonus & relocation assistance eligible.

Facilitates the development and implementation of treatment guidelines, protocols, formulary changes and critical pathways. Facilitates the identification, resolution, implementation, and communication of cost-effective medication use issues in collaboration with the medical staff. Provides clinical consultation and clarification to practitioners and patients and suggests appropriate and cost-effective therapeutic alternatives to medical staff as needed. Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, adverse drug reactions and appropriateness of drug/dose. Prepares and presents drug reviews, MUE criteria, antibiotic sensitivity reports, adverse drug reaction reports and pharmacy intervention summaries to the Pharmacy and Therapeutics Committee. Works under a collaborative practice agreement with provider team.Additional Requirements:
Work schedule: 8-hour shift (Monday – Friday days, various shift.) Education: Doctorate of Pharmacy (PharmD). Experience: Two (2) years of clinical oncology experience. PGY1/PGY2 Oncology residency required. Certification: Licensed Pharmacist in the state of VA. Board certified Oncology Pharmacist or certification obtained within two (2) years of employment.#LI-MF1
