Attemps to engage individuals who are dually diagnosed for group treatment. + Maintains a caseload of 10-12 consumers as assigned acting as the main point of contact for those consumers, ensuring each has a scheduled MD appt. that month, and assisting each consumer in arranging transportation/ planning ahead to keep these appointments, follows up on all missed MD appointments. + At least 80% of services provided to consumers is in the community. + Attends average of 4 weekly treatment team meetings and reports on changes in clinics seen and participates in formulation of next steps for consumers on census. + Available for after- hours (crisis phone) coverage as assigned and provides written notice of planned absence. NOTE: McIntosh Trail Community Service Board conducts background checks on all final candidates. Hiring of applicants is contingent upon satisfactory results of: Fingerprint Based Background Check of the Georgia and Federal Criminal History Record Database Drug Screen Motor Vehicle Report (MVR) for seven years TB testing will be required upon appointment.
