Job Summary
Assesses the patient and prepares an individual Master Treatment Plan and monthly reviews and updates thereafter. Performs Mini-mental status exam, psychosocial assessments, facilitates both individual and group therapy, documenting all interactions in a time frame compliant with company, state, and federal guidelines. Is responsible for contacting any patient who is absent for 3 or more days to determine reason for the absence and when they plan to return to the Program.
Job Responsibilities
Designs a unique Master Treatment Plan for each patient based on diagnosis and issues identified in the assessments. Reviews and updates treatment plans every 30 days.
-Performs Mini mental status exam and psychosocial assessment on all new patients.
-Performs other duties as assigned.
-Provides group and individual therapy and prepares timely documentation in compliance with Company, state and federal guidelines.
Education
Required: Master's degree in Social Work or related field
Experience
Required: Two (2) years of related experience. Work experience may not substitute for minimum education requirement.
Certifications
Required: Must be currently licensed, certified or registered to practice profession as required by law, regulation in state of practice or policy. CPR
