As a Behavioral Health RN, you’ll be responsible for:
âś… Assessing patients' physical, psychological, and environmental needs to develop individualized care plans
âś… Performing initial evaluations and routine follow-up visits to monitor progress
âś… Communicating significant health changes to the Clinical Supervisor, physician, and care team
âś… Educating patients, families, and caregivers on behavioral health strategies and symptom management
âś… Supervising Behavioral Health Aides and LPNs to ensure quality care delivery
âś… Providing emotional support to patients and families, including bereavement care when needed
âś” Current, unrestricted RN license in the state
âś” 1+ years of nursing experience in a clinical care setting
âś” Ability to travel within the coverage area (50%)
✔ Reliable transportation, valid driver’s license, & auto insurance
✔ Associate or Bachelor’s Degree in Nursing (preferred)
âś” Experience in Home Health, Hospice, or Homecare HomeBase (preferred)
