Behavioral Health RN Case Manager

📍 Coverage Area: Worcester

What You’ll Do:

As a Behavioral Health RN, you’ll be responsible for:
✅ Assessing patients' physical, psychological, and environmental needs to develop individualized care plans
✅ Performing initial evaluations and routine follow-up visits to monitor progress
✅ Communicating significant health changes to the Clinical Supervisor, physician, and care team
✅ Educating patients, families, and caregivers on behavioral health strategies and symptom management
✅ Supervising Behavioral Health Aides and LPNs to ensure quality care delivery
✅ Providing emotional support to patients and families, including bereavement care when needed

What You Need to Succeed:

✔ Current, unrestricted RN license in the state
✔ 1+ years of nursing experience in a clinical care setting
✔ Ability to travel within the coverage area (50%)
✔ Reliable transportation, valid driver’s license, & auto insurance
✔ Associate or Bachelor’s Degree in Nursing (preferred)
✔ Experience in Home Health, Hospice, or Homecare HomeBase (preferred)
