SummaryThe Behavioral Health Clinician works in coordination with interdisciplinary teams to organize prepare and deliver evidence based clinical services across the continuum of care Maintains ethical principles and professional standards of practice in adherence to the disciplinersquos professional code of ethicsOutpatient setting of clinical care and populationKey responsibilitiesDirect Patient CareClinician conducts a comprehensive bio psychosocial assessment and establishes a care plan as appropriateProvides direct patient care that may include case management care coordination interdisciplinary collaboration for care transition needs crisis interventionsafety planning counseling and emotional support to patients and families coping with illness trauma or end of life care referral and discharge planningRefers patient to appropriate levels of care and available resources that promote the individualized care planMaintains timely patient centered documentation thatreflect the patient and client systemsrsquo pertinent information for assessment interventions and outcomesin accordance with regulatory licensing requirements and third party agenciesWork whereevery
