Behavioral Case Manager

SUMMARY

Provide Behavioral Non-Clinical Case Management and Administrative Care services for DRC 3/4 soldiers who require behavioral health treatment.

 

ESSENTIAL DUTIES & RESPONSIBILITIES

Responsible for tracking behavioral health status of all assigned Soldiers; including compliance and non-compliance. Responsible for behavioral health charting and inputting all examinations, procedures, and treatment plans. Request profile exams/evaluations including behavioral health exams. Maintain automated referrals. Submit case processing, behavioral health documentation, and profile concerns to military/contracted provider. Attend PHA events as determined by the needs of the state and the individual units. Ensure all medical documentation received from the Soldier and/or their Behavioral Health team is received and placed in appropriated electronic record, as well as the Soldier’s hard copy record. Maintain Soldier behavioral health records in accordance with Army Regulation. Educate Soldiers on resources available to improve Behavioral Health readiness. Maintain HIPAA standards and guidelines. Review and track behavioral health assessment (PHA, SRP) lab results contact SM and educate on lifestyle modifications. Responsible for tracking Behavioral Health status of all assigned Soldiers; including compliance and non-compliance. Upload behavioral health documents of case managed Soldiers into Health Readiness Record. Track Behavioral Health appointments. Maintain regular and punctual attendance. Perform other duties as assigned.

None.

 

MINIMUM QUALIFICATIONS

High School diploma or Associate degree in relevant area of study preferred. At least one (1) year of relevant behavioral health background such as a mental health technician, crisis line worker, or social services case manager. Must obtain and maintain any necessary security access and/or background checks.

 

DESIRED QUALIFICATIONS & SKILLS

Prior or current military Behavioral Health Technician experience preferred. Ability to prioritize, organize, problem-solve, and coordinate daily activities to support effective improvement in individual patients. Knowledge, skill, and ability to use basic office automation and information management equipment and programs (i.e., word processing, database management, email, etc.). Excellent patient assessment skills. Sufficient initiative, interpersonal relationship skills and social sensitivity such that he/she can relate constructively to a variety of patients and healthcare staff from diverse backgrounds. Ability to effectively read, write and speak English in order to communicate with all patients and other healthcare providers. Excellent written communication skills to perform accurate documentation, both written and electronic, of all activity in accordance with requirements.
