Community Health Network's Community Clinics put high quality, convenient primary and acute care throughout all regions of Indianapolis and the surrounding suburbs. + Autonomous model of primary care in a fast paced environment with support of offsite collaborating physician; on-site pharmacist + Clinics are open Monday - Friday 8:30 A - 7:00 P; Saturday and Sunday 8:30 A - 5:00 P + Every other weekend commitment + Closed on Christmas, Thanksgiving and New Year's Day + Competitive compensation and benefit package + Epic; Access to UptoDate Guidelines **Job Duties** Advanced Practice Provider will provide healthcare, including assessment, diagnosis, and treatment of acute and chronic health conditions and wellness preventative medicine.
