Provides telephone care to various patientsProvides other clinical duties as appropriateCompletes and documents accurate comprehensive, episodic history and physical assessmentsPerforms ordering, conducting, supervising, and interpreting diagnostic laboratory and imaging studiesPrescribes treatments, and therapies not limited to medications, diet, supplies, prosthetic devices, and equipmentImplements primary and secondary prevention strategies and management of acute and chronic health problemsProvides clear, concise, accurate, and legible information regarding patient care in the recordFormulates a comprehensive plan of care in collaborating with the patient and interdisciplinary teamFulfills billing and productivity metrics requirements where appropriate and maintains relevant documentationProfessional

Masters of Nursing Degree as APRN-CNP2 years RN experience preferred1 year APRN-CNP experience in the relevant setting preferred1st Assist Required for APRNsKnowledge, Skills & Abilities Required:
Well-developed interpersonal and communication skills.Proficiency with an Electronic Medical RecordNational certification as a CNP from an accredited bodyCurrent Illinois APRN LicenseCurrent Illinois Registered Nurse license.Current Illinois Controlled Substance License within 6 months of hireEligible for DEA registration; Possession of a current DEA Registration within six months of hire and may vary based on practice siteCurrent BLS CertificationCurrent PALS/ACLS certification may vary based on practice site
