Admissions Nurse



Job Summary:

The Admissions Nurse performs pre-admission evaluations for appropriateness for hospice services. Initiates the admission process by educating and informing the patient/family of hospice and its services. Also obtains informed consent. Provides nursing assessment, initial plan of care, and intervention(s)that optimize patient and family directed comfort and quality of life. The Admission nurse will be considered part of the IDT.

Qualifications: A current nursing License in the state, with at least 2 years of experience in home health/hospice. Current Cardiopulmonary Resuscitation certification. Familiarity with a variety of clinical concepts, practices, and procedures. Ability to exchange routine information in an appropriate manner. Good oral and written communication skills. Willingness to work a flex schedule and serve patients/families in the geographic service area that is assigned to you. Detail-oriented with good time management skills.Requires active RN license Responsibilities: Receives assignments from the Director of Nursing or Intake Department for evaluation of appropriateness, sign up, and or assessments for care. The Admissions Nurse will formulate the Interdisciplinary plan of care and communicate to the IDT members including the Medical Director. Obtains data on physical, psychological, social, and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care. Instructs the patient/family on procedures for contacting hospice. Makes initial contact with attending physicians, other hospice team members and other agencies as needed to commence care and use of resources for the patient/family, ie: Pharmacy and DME Maintains daily communication with the Director of Nursing or Intake Department. Turns in all paperwork pertaining to sign-ups and assessments within 24 hours to the Director of Nursing. Maintains confidentiality of sensitive information. Participates in hospice orientation and ongoing education programs. Demonstrates familiarity with policies of the agency and state(s) and federal regulations pertaining to nursing services. Accepts instructions and other related duties as assigned by the Director of Nursing.
