This Opportunity:The primary responsibilities of the Assertive Community Treatment Team Peer Support Specialist are to provide individualized services in the community and promote individual self-determination and decision-making. ACTT Peer Support Specialists also provide essential expertise and consultation to the staff to promote a culture in which each individual’s point of view and preferences are recognized, understood, respected, and integrated into treatment, rehabilitation, and community self-help.

What You'll Do:

•    Carry out duties as a member of the Assertive Community Treatment Team staff, under the direction of the Assertive Community Treatment Team Leader.
•    Participate in comprehensive assessment of psychiatric history (onset, course, and effect of illness, past treatment and responses, risk behaviors, and accomplishments), mental status, and diagnosis; physical health and dental health; use of drugs and alcohol; education and employment; social development and functioning; activities of daily living (self-care, living situation, nutrition, money management); and family structure and relationships.
•    Document participant progress to maintain a permanent record of activity according to the agency's established methods and procedures.
•    Participate in staff organizational meetings and treatment planning review meetings.
•    Contribute to the program's ongoing assessment of participants’ mental illness symptoms and participants’ response to treatment. Share observations and concerns with the staff for consultation and direction and takes appropriate action.
•    Provide direct services to participants on an individual, group, and family basis in the office and in community settings to teach symptom-management techniques and promote personal growth and development by assisting participants to cope with internal and external stresses and move ahead with their goals.
•    Provide work-related supportive services, such as assistance securing necessary clothing and grooming supplies, wake-up calls, transportation.
•    Perform mentoring, problem solving. Offer encouragement on and off the job site.
•    Provide ongoing assessment, problem solving, side-by-side services, skill teaching, support (prompts, assignments, encouragement), and environmental adaptations to assist participants with activities of daily living.
•    Assist participants to find and maintain a safe and affordable place to live--apartment hunting, finding a roommate, landlord negotiations, cleaning, furnishing and decorating, and procuring necessities (telephone, furniture, utility hook-up). Assist and support them to perform household activities, including house cleaning and laundry.
•    Assist and support participants in obtaining adequate financial support (help to gain employment and apply for entitlements). Teach money-management skills (budgeting and paying bills) and assist participants in accessing financial services such as bank checking and savings accounts.
•    Help participants to access reliable transportation (obtain a driver's license and car and car insurance, arrange for cabs, use public transportation, find rides).
•    Assist and support participants to have and effectively use a personal primary care physician and dentist, and eye doctor if they wear glasses.
•    Organize and lead individual and group social and recreational activities to help participants structure their time, increase social experiences, and provide opportunities to practice social skills.
•    Attend and actively participate in meetings and training as required. Maintain certification in all agency, state and federal training requirements.
•    Demonstrate knowledge of and comply with all agency policies and procedures, as well as state and federal statutes and regulations related to specific program areas.
•    Follow service definition guidelines for services being provided.
•    Complete all other relevant responsibilities as assigned by the supervisor.
•    Driving and travel may be required.

Education We're Looking For:High School Diploma (Required)

Certifications We're Looking For:Certified Peer Support Specialist (CPSS) - State Division of Mental Health, Developmental Disabilities and Substance Abuse Services (USA), Drivers License (Valid) - USA

Experience We're Looking For:Mental Health Experience, Substance Use Experience

Schedule:Monday-Friday (8:30am-5:00pm)
