Continuing Healthcare of Boardman is currently seeking an individual with experience in long term care who can assume the role of Activity Director/Social Service Designee.  The primary purpose of this role will be to plan, organize, implement, evaluate and direct both the Social Services and Activity Departments.  

The primary purpose of this role will be to plan, organize, implement, evaluate and direct both the Social Services and Activity Departments.  Responsible for establishing policies and procedures for both departments, maintaining on-going programs for both Departments in accordance with the interests, and the physical, mental and psychosocial well-being of each resident. In addition, must ensure that the medically related emotional and social needs of the resident are met/maintain on an individual basis.
